Customer Agreement

By accepting and signing a Quote (“Order Form”) from AZCOMP Technologies (“AZCOMP”), you (“Customer”) affirmatively consents to the Terms of Service, which is comprised of the following documents, and includes, but is not limited to, the General Terms and Conditions Applicable to All Software and Services (“General Terms and Conditions”), AZCOMP’s Business Associate Agreement, and any applicable Schedules for Software, Services, or Addenda (collectively, “Schedules”). No further acceptance or signature is required by AZCOMP to enforce the Terms of Service. By signing AZCOMP’s Order Form, you represent that you have the authority and right to bind your organization to these terms and conditions.
